去旅行,要帶咩「平安藥」旁身?



今次的事情,很簡單,基本上,內容是這樣的:
一個晴朗的早上,藥房迎來了一個二十多歲的女生,穿著一件背心、一條熱褲、一對涼鞋,背著一個小小的背包,前來藥房,希望配一些平安藥。
據說,這個女生,打算趁著暑假這個黃金機會,聲稱一個人,單槍匹馬,前往日本,準備來一場日本東京五天四夜自由行,吃喝玩樂,便打算在臨行前,買一些平安藥,傍一傍身,希望能夠平安度過這次東京之旅。
嘩……聽著聽著……真的讓人羨慕透頂……要知道,妳在日本觀光的時候,遠在彼岸的我,還要在香港工作……唉……不說了,不說了……
好吧!現在,開始進入正題!
一般而言,平安藥,顧名思義,主要的目的,當然是「平平安安」,居家自用,自然希望「家宅平安」,外出備用,自然便希望「出入平安」。所以,平安藥,主要的用途,簡單說,就是一個「流動的小藥箱」,裡面的藥物,在出門的時候,作為一種備用藥,一直處於待命的狀態,隨時候命,在需要的時候,應一應急,自行解決一些輕微的病症,紓緩一些簡單的症狀,不用尋求當地的醫療援助,最後,確保旅途愉快。
其實,平安藥,作為一種備用品,在大部分的情況下,未必真的派得上用場。說真的,沒有多少人會真的希望使用這些平安藥……所以,面對一個空間不大的行李箱,不難理解,在選擇上,主要是以簡約為主,貴精不貴多,麻雀雖小,五臟卻要俱全,同理,行李雖小,藥物卻要俱全,攜帶最小的藥物,處理最多的病症,同時,準備最常用的藥物,治療最常見的病症。
當然,平安藥,在組合上,主要取決於天時、地利、人和這三個因素,然後,根據相關的資料,啟動「可加可減」機制,作出相應的調整,從而挑選適當的藥物,度身訂造,制訂一份個人的清單,所以,在搭配上,豐儉由人,悉隨尊便。簡單說,不同的時間、地點、人物,自然會有不同的平安藥。

雖然,話是這樣說,無錯,但是,其中一些藥物,俗稱「膽」,大多離不開以下四種:

一、止暈藥

出門,不論前往什麼目的地,只要不能徒步而行、涉水而行的話,自然便需要各種不同的代步工具。實際上,不管是海、陸、空哪一路,有時候,舟車勞頓,可能會出現一些劇烈的搖盪、震盪,在相當程度上,便會大大增加眩暈的機會。
在用藥上,較常用的止暈藥,主要是第一代抗組織胺(First Generation Antihistamine)(例如Cinnarizine、Dimenhydrinate)、東莨菪鹼(Scopolamine Hydrobromide / Hyoscine Hydrobromide)兩種。
在服法上,主要可以分為口服、耳貼兩種。
在使用上,一般的止暈藥,重點是「先發制人」,簡單說,預防勝於治療,因為,相較治療而言,這些藥物,預防的效果較佳。
所以,在用法上,一般建議,在乘坐這些交通工具前,便要開始使用,目的在給予藥物足夠的時間,能夠在體內吸收,不要待到身體開始出現噁心、嘔吐的時候才服用,避免削弱藥效。

二、傷風感冒藥

這個,不難理解。不管是何時、何地,還是何人,不管是居家,還是出門,不管是傷風,還是感冒,兩者同是一些十分常見的病症。所以,大部分的平安藥,總會有一些傷風感冒藥,寸步不離,以備不時之需。
其實,一般普通的傷風、感冒,大多只是一些自限性病症(Self-limiting disease),在正常的情況下,大約在一星期內,便會自行痊癒,所以,這些傷風感冒藥,主要的目的,在紓緩相關的症狀,讓身體自行復原。
有時候,一些藥廠,可能會調配一些全方位傷風感冒藥,俗稱「複方」,一種藥,裡面可能已經蘊含不同的藥用成分,不難想像,多一種成分,便多一種用途,自然便多一種功效,所以,擁有較大的覆蓋性,同時能夠紓緩大部分的傷風、感冒症狀,例如發熱、鼻水、鼻塞、咳嗽、痰液。
在選擇上,基於「輕裝上陣」的前提下,平安藥,當然愈少愈好,少一種藥,便多一分空間,用來擺放其他藥,所以,在這個情況下,為了方便外出,隨身攜帶,這些綜合配方,未嘗不是一個理想的選項,目的在控制藥物的數量,盡量用一種藥,一騎當千,同時解決大部分的症狀。

三、止痛藥

這個,不難理解。不管是頭痛、牙痛,還是經痛,其實,跟傷風感冒藥一樣,同是一些常見的病症。
同時,旅行,其中一項節目,當然是觀光。不管是翻山涉水,還是遊山玩水,觀光,自然少不了遊覽一下景點,欣賞一下名勝,這樣,向左走,向右走,走來走去,便可能會出現肌肉痠痛的機會。
所以,大部分的平安藥,總會有一些止痛藥,不可或缺,以備不時之需。
其中,撲熱息痛(Paracetamol / Acetaminophen),算是一種較常用的止痛藥。
值得一提,在劑量上,一般建議,成人一天不宜服用超過4000mg (4g)。舉例說,以一粒500mg的撲熱息痛而言,這就是說,一天不宜服用超過8粒。

四、止瀉藥

所謂「病從口入」,人在外,不管是吃什麼,還是喝什麼,只要能夠放進嘴裡、吞進肚裡的話,總可能會遇到一些水土不服的問題。
其實,水土不服,顧名思義,就是不服當地水土,所以,在醫學上,稱為「旅行者腹瀉(Traveler’s diarrhea, TD)」。在病源上,主要是透過食物傳染的一種細菌感染,從而誘發腹瀉。
在用藥上,較常用的止瀉藥,主要是Loperamide、Diphenoxylate(大多搭配Atropine)兩種。
但是,在一般的情況下,這種腹瀉大多屬於自限性,簡單說,大多會不藥而癒。一般而言,待到身體透過腹瀉排除刺激物、致病原、毒素後,便可以改善腹瀉的症狀,一般只需要補充所流失的水分、電解質,避免出現脫水、電解質失衡、酸鹼值失衡的情況。
止瀉藥,在相當程度上,目的在緩解因為腹瀉所引起的不便。
當然,如果腹瀉持續一段時間,或者便中帶血或黏液,這時候,便應該盡快求醫處理。
至於,在包裝上,基於「輕裝上陣」的大前提下,排裝藥物,可能是一個較理想的選項,在攜帶上,一般較方便,而且,背面的鋁箔紙,大多會附上藥物的名稱、劑量,較容易辨認,所以,較適用於作為一種平安藥。
還有,在藥量上,取決於旅程的長短,一般建議,採取「寧濫勿缺」的策略,只有多,沒有少,如果是五天四夜的話,至少,便準備五天的藥量,便是了。

最後,這個女生,因為家裡還有一些止痛藥、傷風感冒藥,所以,只是買了止暈藥、止瀉藥,除了問多少錢外,還補上一句話:
「唔……還有沒有其他東西?」
其實,答案,倒是有的。
說真的,出入平安,除了向內看外,還要往外看,簡單說,內憂、外患,同時兼顧。
這話怎麼解?
有時候,不管是室內,還是戶外,不管是居家,還是出門,總可能會發生一些跌、踫、撞、擦的情況,便可能會出現受傷的機會,從而形成傷口。
所以,除了平安藥外,最理想的做法,還是準備一些藥水膠布,用來處理一些簡單的傷口。
問題是,藥水膠布,算是一種常用的傷口護理用品,在絕大部分的家裡,不難發現,所以,一般而言,只需要在家裡拿一些藥水膠布出來,放進行李箱裡,便是了。
所以,藥罐子便答道:
「如果家裡沒有藥水膠布的話,便買一盒,拿一些出來,備用傍身。」
還有,出門前,請準備平安藥;出門後,請準備手信……

The slippery polymath of postwar German art, currently subject of an outstanding retrospective at New York’s Museum of Modern Art, spent years designing new windows for Zurich’s thousand-year-old cathedral, which was stripped of its ornaments during the Reformation. While five of the windows depict biblical scenes, the most breathtaking are Polke’s abstract compositions made of thinly sliced agate, which break up the church’s solid walls with pools of light.
